This study sought to explore the differential diagnosis debate. A phenomenological qualitative study with eight co-researchers, clinically diagnosed with psychosis, explored how they derived transformative meaning from their extreme states and identified factors influencing their meaning-making processes. Through the potential of heuristic inquiry to advance social justice, the research aimed to contribute to the creation of a bridge between the fields of transpersonal psychology and psychiatry. Semi-structured interviews were conducted with eight co-researchers. The findings suggest that all co-researchers underwent a positive personal transformation as a result of their experience, due to the meaning they harnessed from it. Reframing their experiences as a deeply meaningful process hermeneutically liberated co-researchers. Findings are discussed in relation to previous literature. A proposal is made for the field of transpersonal psychology to abandon the concept of differential diagnosis to align with the leading-edge of mental health research in order to support the emancipation of suffering for the collective. Limitations are discussed and future research suggested, including a repetition of this study with non-white Western participants to harness indigenous insights, and an explorative study into the efficacy of transpersonally-informed approaches in the clinical treatment of psychosis. The study concludes by suggesting that a transdisciplinary, transcultural approach could provide an evolved, multi-perspectival lexicon for extreme states, interweaving indigenous wisdom, decolonised language and perspectives, and trauma-informed clinical practices.
